Here is a checklist to assist with the completion of the Personal and Work History Document for Reserve Deputy Sheriff Applicants.
The following items address areas of the application from where errors or omissions commonly occur. Many sections of the application are easily understood, requiring no special instructions.
1. Is your application typed or neatly printed (using blue or black ink)?
2. Read you filled out application section by section to make sure you have answered all the questions to your satisfaction. If a section does not apply, did you put the initials DNA at the beginning of the section? (You do not need to write DNA in every space for the section.)
3. Are your answers complete and are they understandable to anyone reviweing your application? Provide supplemental pages for answers requiring lengthy responses.
4. In Section I, Personal Information (page 3 of 17), did you date the application and did you write "Reserve Deputy Sheriff" as the position applied for?
5. In Section I, Current Residence (page 3 of 17), if renting did you give the name, address, and telephone number of the person to whom you pay rent or are visiting?
6. In Section II, Family History (page 4 of 17), are your relatives listed in the designated order?
7. In Section IV, Residence Information (page 6 of 17), did you leave any unaccounted for gaps in time? There should be no gaps.
8. In Section V, Employment Information (pages 7-8 of 17), are there any gaps in time? There should be no gaps. You can provide a supplement page to explain unusual employment situations.
9. In Section VII Military Information (page 10 of 17), have you provided a written explanation for any "yes" response?
10. In Section VIII, Financial Information (pages 11-12 of 17), have you provided a written explanation for any "yes" response?
11. In Section XI, Traffic Information (page 15), have you provided complete insurance information? Remember that state law requires you to have liability insurance for all vehicles you own.
12. In Section XII, References (page 17 or 17), did you provide at least two references that are approximately your own age? Have you provided complete address information (no P.O. boxes) for your references including Zip Codes?
13. Be sure you have signed and dated your application. (Page 17)
14. Remember to bring two photographs (passport size). Print your names on the back of each photograph.
15. Be sure you bring your autobiography. It must describe your life from birth to present and include any traumatic experiences, three high points in your life and three low points. It should be hand printed on one side of at least five sheets of lined white paper.
16. Also bring two single-paragraph compositions on separate sheets of line white paper that describe three of your character strengths and three of your character weaknesses.
Be prepared to return with your completed application and the documents at the next orientation meeting.

To start the background investigation process, the following must be submitted:
- completed application
- two passport style photos
- applicant autobiography
- description of applicant's three strengths and three weaknesses
If you are submitting your application to an LASD facility other than Reserve Forces Bureau, you must mail a copy of the first page of the appliation form to:
